USES AND DISCLOSURES OF HEALTH INFORMATION:

We may use and disclose your information about you for treatment, payment and legal procedures.

A consent form is available to you with your new client intake packet. You chose with whom your information may be shared. Your authorization will allow us to disclose any of your information that you may choose to share.

Healthcare Operations: We may use and disclose your health information in connection with our healthcare operations. This includes quality assessment and improvement activities, reviewing the competence or qualifications of healthcare professionals, evaluating practitioner and provider performance, conducting training programs, accreditation, certification, licensing or credentialing activities.

Treatment: We may disclose your information to a healthcare provider providing treatment to you.

Payment: We may use and disclose your information to obtain payment for services rendered to you.

Required by Law to avert a serious threat to health or safety: We may use and disclose necessary medical information about you to prevent a serious threat to your health and safety or to the health and safety of the public or another person. Any disclosure to an individual would be to said individual able to help prevent the threat.

Abuse or Neglect: We may disclose your health information to appropriate authorities if we reasonably believe that you are a possible victim of abuse, neglect or domestic violence or the possible victim of other crimes. We may also disclose your  information to the extent necessary to avert a serious threat to your health or safety or the health or safety of others.

 

QUESTIONS AND COMPLAINTS:

If you would like to request additional information about our privacy policies or if you have questions

or concerns, please contact our office. If you are concerned that your rights have been violated,  you may file a complaint to our office or the Secretary of the United States, Department of Health and Human Services. All complaints must be submitted in writing.
